Registered Nurse- Home Dialysis Nurse Job Responsibilities: •Plan and provide education, training and clinical support for home dialysis ESRD patients •Provide education about treatment options •Train the patient/care partner in techniques to manage their health status, in home therapy techniques and in problem solving •Provide problem solving, trouble shooting and counsel to the home patient/care partner by phone •Periodically conduct home visits •Analyze patient data including status of access, lab work, patient assessments •Make recommendations to the physician for patient care •Work with the team to develop short and long term care plans •Participate in the care center's QA/CQI process •Provide documentation for charges and order/monitor home supplies Requirements: Education: Bachelor's degree in Nursing, Associate degree in Nursing or Nursing Diploma program. Current RN license in the state of practice; CNN desirable.
