Description The primary function of this position is to improve outcomes of patients new to hemodialysis by collaborating with staff and providing guidance, education and support to the patient to enable optimal adjustment to new medical needs and lifestyle changes. Operates as a case manager for patients new to dialysis treatments by incorporating case management and patient education activities into the first 120 days of treatment. Oversees the planning, implementation and evaluation of the RightStart Program at the designated facilities. Acts as a resource to dialysis staff and other healthcare professionals within the assigned clinics. Assists in facilitating changes in patient care delivery as needed. Supports FMCNA's vision, mission, core values and customer service philosophy. Adheres to the FMCNA Compliance program, including following all regulatory and FMS policy requirements. Requirements Nursing degree or degree in dietetics/nutrition preferred. Current RN License required to practice professional nursing in applicable states or current registration as a Registered Dietitian to practice dietetics/nutrition. If the state has licensure or certification, current license or certification to practice dietetics/nutrition in the states(s) or Current Practical Nurse license to practice practical nursing in the state(s) At least 3 years previous dialysis patient care experience is required. Experience in a role related to patient education and/or case management is preferred. Must exhibit management/leadership skills and be able to work with and influence staff in the clinics. Must have knowledge of teaching-learning process, principles, and methods of adult education. Understanding of renal and diabetes disease process and current management practices. Excellent written and verbal communication is necessary.
