Social Worker Job Responsibilities: •Provide psychosocial counseling, information referral and treatment planning for patients and their families to maximize the social functioning and adjustment of ESRD patients •Work with a team of clinicians in developing and monitoring short and long-term care plans •Conduct psychosocial assessments and counsels patients on life style issues related to ESRD •Act as an advocate on behalf of the patient and family •Provide patient education and/or support groups •Participate in the care center's QA/CQI process Requirements: Education: Masters Degree in Social Work (MSW); state license in Clinical Social Work if required Experience: Minimum of two years of experience as a social worker in a health care setting For more information on our company or to apply online, please visit our website: www.renaladvantage.com Renal Advantage Inc. is an Equal Opportunity Employer |
