Description
Responsible for the support, delivery, and compliance of all company policies and procedures. Promote and assist with the compliance to OSHA programs in order to maintain a safe, organized working environment. Provides safe effective delivery of CKD patient care and education in compliance with requirements outlined in the CKD Services Operations Manual, Kidney Options Staff Manual, facilities procedure manual as well as regulations set forth from the corporation, state and federal agencies. Provides advanced nursing care for the CKD patient with nephrology and co-morbid medical problems in collaboration with the nephrologist. Utilizes resources cost-effectively. Evaluates the outcomes of CKD Services patient care. Directs and collaborates as is appropriate with the MD, CKD RN, Dietitian, Social Worker and patient care technician in providing CKD Services (MD only can order MNT services.) Manages /is responsible for the CKD Services program where appropriate. Responsible for the CKD Services patient medical record. Coordinates/maintains the CKD Services CQI program. Participates in clinical studies. Responsible for reporting and documenting all new or unusual incidents, clinical variances, complaints, or problems to facility management if applicable. Provides advance nursing care for the CKD patient with nephrology and co-morbid medical problems by obtaining the history of the problem, performing the necessary physical exam, orders and interprets test results and ordering treatment and medications as needed. Utilizes resources cost-effectively. Performs patient history/physical exams and ongoing patient care/coordination of the CKD patient. Advance nursing care includes assessment, physical exam, writing orders, testing, evaluating, prescribing, counseling and providing education regarding all aspects of CKD patient care including utilizing Kidney Options. Seeks appropriate consultations and/or referrals for the CKD patient in the event that a problem or abnormality is beyond the normal scope of nurse practitioner practice. Communicates relevant patient data verbally and/or in the medical record. Maintains patient confidentiality. Performs all CKD Services as appropriate for the CKD patient and MD referral. Insures CKD patient admission packet is completed on every CKD patient. Insures CKD patient admission packet is completed on every CKD patient. Accurately completes/faxes the CKD Services billing log the day of service. Assures patient's need for immediate and emergent care is handled appropriately. Tracks patient outcomes per FMC-NA CQI policies.


Requirements
Graduate of accredited School of Nursing (RN). Graduate of accredited School or Nursing Nurse Practitioner Program. Master's degree in Nursing if required by state law. Current state specific licensure. Previous experience in nephrology nursing preferred. One year nurse practitioner experienced, nephrology practice preferred. State authorization to practice in the expanded role. DEA Licensure. UPIN number or eligibility. Maintains BCLS certification. Maintains state professional licensure requirements. American Nephrology Nurses Association membership preferred.
