GENERAL PURPOSE OF THE JOB
Provide skilled, appropriate, efficacious professional nursing care to patients in the ambulatory suite and hospitals. 
  
ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive.
  • Conduct comprehensive initial assessment of patient/caregiver to determine the current medical and psychosocial status, appropriateness of care and criteria for acceptance.
  • Establish a patient specific plan in conjunction with the patient/caregiver, the primary physician, Director of Nursing, and other appropriate health care providers (i.e. pharmacist).
  • Provide direct patient care as outlined in the patient specific plan of treatment and physician's orders.
  • Provide care and administer medications and procedures according to organization's policies, procedures, and protocols.
  • Provide all components of care in a professional, goal oriented, patient specific manner, functioning within the Nurse Practice Act and participates in the inter-disciplinary care planning process.
  • Counsel and instruct the patient and family or caregiver in ways to assist, meeting the patient care needs.
  • Document all components of care rendered to organization's policies and procedures.
  • Provide both verbal and written communications to the patient's physician.
  • Communicate any changes in the patient's condition, appropriate supervisor, other members of the inter-disciplinary care team.
  • Participates in organization's Improving Organizational Performance (IOP) program.
  • Comply with organization's personnel policies and procedures.
  • Participate in initial and ongoing in-service training programs.  Obtain continuing education as needed to maintain current license and competencies and proficiencies.
  • Assist in supervising and orienting other nursing personnel.
  • CRNI within two years from the date of hire.
  • Other duties as assigned.
MINIMUM QUALIFICATIONS
(Education, licenses, certifications, and experience required to fulfill the essential duties, include computer skills as required):
·      Registered nurse with a current, valid nursing license.
·      Minimum one (1) year of experience as a professional nurse.
·      Experience in home health is desirable.
·      Experience in the provision of home health is desirable.
·      CRNI preferred.
·      Current CPR (cardiopulmonary resuscitation).
·      Valid Driver's License.
·      On-Call Availability
·      Able to pass any background screenings as may be required by law and regulation.

PHYSICAL DEMANDS/HEALTH STATUS REQUIREMENTS
Physical requirements can vary. These must be reviewed with management.
  • Ability to lift 30 pounds.
  • Proper immunizations current.
  • TB screen.
  • Other state required health tests allowable by law.
  • Physical demands (per worksheet).
  • Hepatitis B vaccine or signed waiver.
