GENERAL PURPOSE OF THE JOB
The Infusion Coordinator is responsible for coordinating patient's admission to home care services in a smooth systematic process, as well as, arranging for necessary ancillary services so that they are complimentary and non-duplicative.  The Infusion Coordinator acts as a liaison for care coordination with staff within the organization or with outside agencies. 
 
ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive.
  • Facilitate the effective operational work flow of new admissions to home care services.  This includes, but is not limited to, the supervision of the intake, coordination of services, assignment of a primary nurse/case manager or the coordination of the nursing services with an outside nursing agency, and review of the discharge criteria and/or other patient-related services, i.e. equipment.
  • Facilitate the effective operational work flow of ongoing patients on home care services.  This includes, but is not limited to, overseeing that care coordination is being provided by ensuring that inter-disciplinary case conferences are occurring (as appropriate), that information is communicated about the patient's status to both the internal staff as well as any outside organizations involved in the patient's care, that information is documented, that care plans and orders are updated.
  • Ensure that current and accurate communications with the patient's physician, outside organizations, and any additional health care providers occur in a coordinated manner.
  • Maintain liaison with referring sources, i.e., outside nursing agency, the discharge planner, or the physician, regarding patient outcomes.
  • Conduct community educational programs as deemed necessary by the nursing and/or pharmacy staff for outside organizations or internal staff.
  • Use of clinical knowledge and skills for the identification of company growth.
  • Participates as a part of the branch nursing staff when applicable and must participate in the On-Call rotation schedule.
  • Responsible for providing all components of care in a professional, goal oriented, patient specific manner, functioning within the Nurse Practice Act and follows the Policy and Procedures of HomeChoice Partners when rendering any and all nursing care.
  • Provides direct patient care as outlined in the patient specific plan of treatment and physician orders.
  • Participates in multidisciplinary care team planning with relation to patient care.
  • Contributes to the positive culture of HomeChoice Partners and demonstrates the ability to support HomeChoice Partner's mission and values.
  • Participates in Improving Organizational Performance (IOP) activities.
  • Other duties as assigned.

MINIMUM QUALIFICATIONS

(Education, licenses, certifications, and experience required to fulfill the essential duties, include computer skills as required):
  • Current license to practice as a Registered Nurse (must be in good standing) in state of practice or Nursing Compact Licensure (NLC).
  • Maintains active nursing licensure in the state of practice and or within the Nurse Licensure Compact (NLC).
  • Maintains any and all CEU requirements for licensure if applicable
  • Maintains and updates CPR Certification
  • CRNI examination within two years of hire
  • Extremely organized and has very effective time management skills.
  • Excellent written and verbal communication skills.
  • Excellent follow through skills.
  • Excellent inter-personal skills.
  • Must be familiar with general use and functions of a computer which includes but is not limited to internet, e-mail, windows, and excel.
  • On-call availability.

PHYSICAL DEMANDS/HEALTH STATUS REQUIREMENTS
Physical requirements can vary.  These must be reviewed with management.  However, in general, the position required the following physical activities:
  • Ability to lift 30 pounds.
  • Proper immunizations current.
  • TB screen (as required by policy).
  • Other state required health tests allowable by law.
  • Hepatitis B vaccine or signed waiver.
