Ensures quality and efficient customer service by leading an assigned team of UltraCare Case Managers, assisting the supervisor with the team's daily operations and acting as a mentor to other team members by providing subject matter expertise and informal coaching when appropriate. Coordinates the daily responsibilities and work load of the assigned team assisting the Supervisor with the assignment of tasks and duties. Tracks productivity of assigned team, working with Case Manager team to ensure daily goals are achieved and that each Case Manager's workload is fully balanced. Identifies and recommends methods to improve area operations, efficiency and services, providing input for tracking mechanisms and reports for performance indicators. Assists with the Service Observe Process to ensure quality and accuracy including auditing of patient records in the web based computer application, monitoring assigned calls, providing coaching and completing related paperwork. Assists UltraCare Case Manager Supervisor's evaluation of staff knowledge and understanding to identify areas of improvement regarding their specific role, providing the appropriate support to facilitate understa Requirements RN required with active unrestricted nursing licensure in practicing state. BSN preferred. 2 years of Nephrology Nursing (preferred) or Case Management required. 6 months to 1 year experience as an UltraCare Case Manager required. Experience in a role related to patient education and/or case management is preferred. Must exhibit management/leadership skills and be able to work with and influence staff in the clinics. Must have knowledge of teaching-learning process, principles, and methods of adult education. Understanding of renal and diabetes disease process and current management practices. Excellent written and verbal communication is necessary. Excellent and professional telephone skills. Ability to function as a part of an integrated team. Adequate presentation skills needed to prepare and deliver presentations to small groups telephonically or in person. Ability to develop positive relationships with patients, families, physicians, staff and other disciplines. Must be self-motivated, organized and able to plan a diversity of activities, and work independently.
